Plan Type
- Single: $275 (Total Annual Cost)
- Dual: $550 (Total Annual Cost)
- Family (3): $800 (Total Annual Cost)
- Family (4): $1000 (Total Annual Cost)
($250 for each additional member)
The dual plan is for parent/child or husband/wife ONLY.
Family plan includes family members and children who are enrolled full time in college until the age of 23 or children who are not enrolled in college until the age of 18.

- Panorex X-ray (1 every 3 years): 50% (Member Discount)

Preventive
- Cleanings (2 per year): 100% (Member Discount)
- Additional cleanings per year: 30% (Member Discount)
- Fluoride Treatment (2 per year to age 18): 50% (Member Discount)
- Adult Fluoride Treatment: 50% (Member Discount)
